Enrollment Application

To begin the enrollment process, please download the Enrollment Contract to your computer by right-clicking on the PDF icon and choosing “Save Target As...” or “Save Link As...” from the menu. Then print it out and follow the included instructions for submission.

Creative Care Contract

To enroll in Creative Care, please download the Contract to your computer by right-clicking on the PDF icon and choosing “Save Target As...” or “Save Link As...” from the menu. Then print it out and follow the included instructions for submission.

Liability Release Form

In consideration for being accepted by St. Cyprian’s Episcopal School for participation in one or more off-campus trips during the academic year, you will need to complete and submit the Liability Release Form. Please download the form to your computer by right-clicking on the PDF icon and choosing “Save Target As...” or “Save Link As...” from the menu. Then print it out and follow the included instructions for submission.

Acceptable Use Policy

St. Cyprian’s Episcopal School believes in Technology for Tomorrow. As such, we provide several programs which allow students to share their work publicly. This Policy Agreement is used to facilitate the sharing of that information. Monthly Tuition Agreement

To enroll in St. Cyprian’s monthly tuition agreement, this Authorization for Pre-authorized Payments form needs to be completed. Please download the form to your computer by right-clicking on the PDF icon and choosing “Save Target As...” or “Save Link As...” from the menu. Then print it out and follow the included instructions for submission.
